Services

Specialist Support Coordination is support coordination that is provided by a professional with an allied health background, Psychosocial, social work, or other appropriate expertise. This is funded for complex participants who have exceptionally high needs or are in a crisis situation.

Often when Specialist Support Coordination is funded, there are matters that are very time-sensitive and where the participant may face additional and high-risk barriers to support.

Specialist Support Coordination includes all the activities outlined in ‘Support Coordination’, but addresses situations where it is appropriate to have a Specialist deliver Support Coordination-style services, necessitated by specific high-level risks in the participant’s situation.

Working with children

Services

The Early Childhood Early Intervention (ECEI) service is funded by the National Disability Insurance Scheme (NDIS) and can offer a range of supports for children (aged 0-6 years) with developmental delay or disability and their families.

The types of supports that can be provided through ECEI include:

  1. Information
  2. Referral to mainstream or community services
  3. Looking at what is needed to achieve the goals set by parents and their children
  4. Short term help to make sure kids are doing well in their early education setting
  5. Where required, assistance to access funded support through the NDIS

Our service can work with children of all ages who live in NSW and have a disability or a developmental delay which is the result of an impairment and causes substantial functional limitations. We work very closely with their caregiver and provide therapeutic services.  The SSP has been found to have significant gains in the development of children with disabilities.

Participation

Services

We provide participants with the reasonable and necessary supports to take part in daily activities to encourage participation in the community.

A large part of this goal is to get participants to be more active in social and community programs. Our professional NDIS support workers can facilitate certain social gatherings and support and accompany you to events.

Community and social access relate to assisting with or supervising a participant to engage in community, social, recreational, or economic activities. These supports can be provided in a range of environments, such as in the community or a centre.

Some of these services include:

  1. Support to community activities such as attending a community youth group.
  2. Support to study, learn or volunteer for different programs.
  3. Going to the mall, shopping, or eating a meal
  4. Going to see a movie or a concert and fitness activities.

Daily Care and Life Skills

Services

We understand and appreciate the level of complexity that people with disabilities face when tackling day to day tasks. For instance, it takes extra time and effort for a person with a disability to wake up in the morning, dress, prepare and eat breakfast, go to work, and overcome daily challenges.

That’s why we want to help you manage these challenges by offering a wide range of training and development activities. We help build your capacity by developing your skills, knowledge, and networks to enable you to manage your daily life smoothly. Our team offers training on general daily tasks and life skills including.

  1. Nutrition – our team will help you shop for groceries, plan for meals, and prepare healthy and delicious meals.
  2. General cleaning – basic household tasks including laundry, house cleaning, and gardening.

Our team will support you with the skills required to live as autonomously as possible. We also respect your needs, so let us know what general skills you want to learn, and we train you. We make sure our participants are living in a safe, secure and healthy environment.

Transport

Services

Transportation is an important part of everyday life. You may need it to get to work, place of study, medical appointments, or any other commuting needs. We offer necessary support with transportation, which assists participation in community, social, economic, and daily life activities.

Our support workers will assist participants in traveling with significantly greater flexibility, whether that be by bus, train, car, or ferry. We will make sure we provide you with a safe and reliable support worker who is a well-trained licensed driver in our own vehicles.

If you need assistance with travelling to:

  1. Your GP or any other medical appointments
  2. Any educational facility
  3. To access your local community

Transitions and Psychosocial recovery coaching

Services

NDIS provides the resources to help people living with disability connect with supports and services. Our Support Coordinators can unpack the supports in your NDIS plan. We help you to independently access the services that suit you.

Psychosocial Recovery Coach

Psychosocial Recovery Coaches help people diagnosed with psychosocial disability to access the more specific supports they need to get the most from their lives. We provide coaching in recovery and decision-making skills to work towards goals, such as more independence and help develop personalised Recovery and Crisis Management Plans. Our team use skills and experience in mental health services to ensure you receive the best support to get you to where you want to be.
